    That's a serious accident, anyone know exactly what happened?

    Yep. Somebody screwed up. Somebody died.

    The one paragraph news (translated) reports don't give a bit of information.

    More info found: the yacht was chartered from a San Marino charter company from a Russian, they started from a marina in Rimini they stopped in Brindisi and the yacht left the marina of Brindisi at 5:30 am, the condition of sea and the wind were very bad, after 1 hour the engine stopped for a problem, the launch the MAY DAY and a boat try to help them, but no way, due to the waves and the wind the yacht crash and before the 3 people on board were able to left the boat, but the captain had an heart attack and died during the transfert to the local hospital.
