    It's an interesting document with the intent stated as learning lessons. It's a discussion of 23 major marine accidents. Interestingly enough only one involved a pleasure yacht and it was the fire of the OA at Roche Harbor Marina, caused by an electrical fault of unknown cause in the forward accommodation area. One reminded me of the Miami bridge closing on the yacht which perhaps will be in next year's document. This one was the Allision of the Bulk Carrier Herbert C. Jackson with the Jefferson Avenue Bridge near Detroit. Cause was "the intoxicated bridge tender's closing of the drawbridge as the vessel began it's transit through the open bridge span." Most of the accidents are fishing vessels and towing vessels. The issues cited were Captains not understanding the control systems, passengers on stairwells during docking and undocking, lack of proper maintenance on wooden vessels and inadequate crew training.

    Thanks for posting this outstanding report. Regardless of vessel purpose, the common denominators that apply to luxury yachts of all sizes and types for damage prevention are proper maintenance and owner/operator familiarity. Accidents happen but much of the damage and many of the lives lost could have been prevented with more meticulous inspections of equipment, improved maintenance, and a better choice of captains at the helm.
