Guest Questionnaire?

Does anyone have a questionnaire prepared for guests which includes things like food likes/dislikes, normal waking time, movie preferences, snacks, etc?

I can make one up but thought a comprehensive one might already be out there.

Hey guys!

I could go stay at the Shangri-la in Hong Kong for $300 a night. I'd be able to point at a fish swimming in the tank and minutes later, I'd be eating that fish.

Are you saying I'd be better off if I chartered your yacht for $20,000 a night?!

I could go stay at the Shangri-la in Bangkok for $300 a night. I'd be able to say that I had the most appaling back-ache and minutes later, I'd have a delectable young lady soothing away every ache imaginable.

I could go stay at the Shangri-la in Port Dickson for $300 a night. I'd be able to wander off to a nearby street hawker and have charcoal-barbecued satay with peanut sauce minutes later.

A Captain I was working for on a charter yacht always use to prep the crew for charter by starting out with this statement:
"The guess that are coming onboard can afford to vacation any where in the world they choose and they have decided to vacation with us......."
